| For info on Midwivery services, to schedule and appointment or questions that can't wait until your next appointment, please call the office. Due to the high demand for midwifery services, it is not unusual for our practice to become full very quickly. If you are interested in receiving midwifery care, you will need to fill out a CLIENT INTAKE FORM. We review intake forms several times per week and if there is a spot available for care during the month you are due, you will be contacted for an appointment with us. Due to the high number of intake forms received each month, you will not be contacted unless there is a space for you. If you have not been contacted by your 20th week of pregnancy, you will be removed from our wait list unless otherwise requested. It will be the midwives discretion to remove anyone from care following your first visit if for any reason, medical, or otherwise, you are inappropriate for our low risk care. |
